We are looking to do screening and confirmations on urine and some saliva samples that will be ordered by mostly pain management doctors. We are starting to research equipment and are considering the refurbished API 4000. We have also contacted the local reps at Agilent and Waters for recommendations. I am looking for some advice regarding what company to go with and what questions we should be asking. Would appreciate any insight. Thanks.

I am a devoted Agilent user. For my money, their equipment is the best. I have only rarely had problems with any of their components. If it's a GC system, I wouldn't look anywhere else. I'm not as familiar with their LCMS systems.
We have an Agilent LCMS in our department but in my life here, we have not used it to its full capability. So, I can't comment on its operation.
Good luck in your pursuits.

Agilent's system to do the pain management work is unparalleled. If you are novice LC-MS-MS users I DEFINITELY would not go with a used system. I have an AB/Sciex system that I love, but I am hardly a novice. The Agilent system is not cheap, but if you're hooked up with a pain management team then you'll have the workload to pay it back.
The work you're trying to do is quite difficult. You'll need a real chromatographer on your team.
